Provide Resources:
- Provide expert consultation and monitor for physical and mental changes
- Assist with planning and goal setting
- If additional medical services become necessary, provide referral
- If a move becomes necessary, provide support from beginning stages to “sold”
- Provide care and support after a move has taken place, even 24/7 if necessary
